The Health Equity Council is committed to improving the health and well-being of all Bernalillo County residents through collaborative efforts and targeted strategies. Our approach focuses on understanding and addressing the diverse needs of our community to create lasting health equity.

  • Community Health Profile: We develop a comprehensive Community Health Profile that maps and evaluates local health resources, needs, and characteristics. This profile serves as the foundation for data-informed decision-making and strategic planning.
  • Identifying Priorities: We work with community members to identify and prioritize key health issues affecting individuals, families, schools, and neighborhoods. This ensures our efforts are aligned with the most urgent community needs.
  • Building Partnerships: Through strong partnerships with individuals, community organizations, and agencies, we promote health awareness and deliver educational programs to empower residents with knowledge and resources.
  • Action-Oriented Strategies: We design and implement action plans aimed at driving systemic changes to meet the health needs of all Bernalillo County residents. Our goal is to create an inclusive, responsive health system that fosters long-term well-being.

The 2022 Community Health Improvement Plan (CHIP) provides a strategic framework for advising the City of Albuquerque on key health issues. This plan is designed to guide policy development, promote health as a priority in all areas of governance, and establish a robust public health system. The 2022 CHIP serves as a blueprint for achieving health equity by embedding health considerations into policies and services that affect the daily lives of our residents.ould also serve as a guiding document for the policy-making system to view health as central to all services, to build a public health system, incorporate health in all policies, and develop strategies to address the health needs of the community.
